Overview
The ICLS team will support you to set up your ICLS home (if applicable) and develop an Individual Funding Plan outlining clear and achievable goals to meet you wherever you are on your path to recovery. Your ICLS Recovery Support Worker will work with you to build capacity and confidence assisting you to overcome any obstacles as they arise. Some of the areas that we provide support include:
- Maintaining your tenancy;
- Engaging with your healthcare providers (e.g. GPs);
- Developing/Increasing your social connection;
- Managing physical health;
- Managing mental health and wellness; and
- Finding study, volunteer work, or employment opportunities.
We understand that family, guardians and/or friends are an important part of life and need to understand what is happening. At your request, and with your consent, important family members and/or friends can be involved in shaping your ICLS future. This can include their involvement in the planning, delivery and review of services, access to resources, support and information, as well as being listened to and heard.
